How long will my treatment session take? How many sessions will be required?

Initial assessments will last 60 minutes. During this time, your clinician will assess your issue and develop an individualized treatment plan. All subsequent visits will be 30 minutes in duration.

The amount of sessions will depend on the diagnosis provided by your clinician. Since each treatment plan is tailored to your specific needs, the rehabilitation process won’t be known until after the initial assessment.

Your clinician will provide you with all of the details of your treatment plan at this time.

How do I know if I’m covered? How much does it cost?

If you have insurance through your workplace, a family member or a private health plan, you’re most likely covered. Don Valley Health & Wellness – Sports & Physiotherapy Rehabilitation Centre can help you figure out your benefit plan – please call us at (416) 466-4438 or simply bring your insurance card to your first appointment.

Generally, the cost of most services is covered through your health benefit plan. If you want to be sure, please call (416) 466-4438 or e-mail info@dvhealthandwellness.com with your specific information and we’ll let you know exactly what you’re covered for.

Do I require a referral from my doctor?

A referral is not required in order to receive any treatment. However, some insurance companies may require a referral in order to provide reimbursement. If you require further information, please call (416) 466-4438 or ask a Don Valley Health & Wellness – Sports & Physiotherapy Rehabilitation Centre clinician or other member of staff.

Are the clinicians registered?

Each of our clinicians are licensed to practice their respective disciplines in the province of Ontario. Additionally, our clinicians actively strive to advance their professional development through continuing education courses and certifications.

How do I access services at the clinic?

You can access service by telephone, e-mail, fax or walk-in.

Call (416) 466-4438 Monday to Thursday from 9:00 am – 7:00 pm or Friday between 9:00 am – 4:00 pm to speak to a receptionist and schedule an intake appointment with an appropriate clinician.

Send an e-mail to info@dvhealthandwellness.com and include your contact information, best times to reach you and a brief outline of your health condition, and we will respond within one business day.

Fax

You can also fax a request to (416) 466-4452 and ask us to call or request an appointment. Please include your contact information, best times to reach you and a brief outline of your health condition, and we will respond within one business day.

Walk-in

If you’re in the neighborhood, please drop in during office hours Monday to Thursday from 9:00 am – 7:00 pm or Friday between 9:00 am – 4:00 pm to speak to the receptionist about scheduling an intake appointment with the appropriate clinician.

What are your hours of operation?

Don Valley Health & Wellness – Sports & Physiotherapy Rehabilitation Centre is open Monday to Thursday 9:00 am – 7:00 pm and Friday 9:00 am – 4:00 pm.

Where are you located and how do I get there?

Don Valley Health & Wellness – Sports & Physiotherapy Rehabilitation Centre is located at 658 Danforth Avenue, Suite 200, at the northeast corner of Pape and Danforth. Enter the lobby of the building at 658 Danforth Avenue and take the elevator to the second floor to Suite 200, next to the courtyard fountain.

Motor Vehicle & Parking

The clinic can be reached either by Pape Avenue or Danforth Avenue.

Parking

In addition to street parking, the nearest ‘Green P’ public parking lot is located on the west side of Pape Avenue directly north of Danforth Avenue.

Subway

Located just steps from the Pape subway station on the Bloor-Danforth (Green) TTC line. Leaving the station, walk one block south on Pape to Danforth Avenue and turn left on the Danforth. The clinic is in the first building on your left at 658 Danforth. Enter the building and take the elevator to the second floor; the clinic is located next to the fountain in Suite 200.

Where can I park?

How much health plan coverage do I have?

Your Don Valley Health & Wellness – Sports & Physiotherapy Rehabilitation Centre clinician can help you determine your extended health coverage benefits and the clinic will perform a free coverage check for you on your first visit.

Is my partner or family member covered?

The Don Valley Health & Wellness – Sports & Physiotherapy Rehabilitation Centre will review your coverage and, provided your insurance plan extends to family members, they’re most welcome to use clinic services.

What should I wear to my appointment?

We recommend that you wear comfortable clothing as you may be participating in exercise, depending on your specific treatment. Additionally, try to keep any injured areas accessible.

For example, if you have a shoulder injury, wear a loose fitting t-shirt in order to provide your clinician with the ability to comfortably work on the area if necessary.

Languages spoken?

The clinic employs a regular full-time team of dedicated clinicians and currently has staff members who speak English, French, Greek, Cantonese or Mandarin. Please let us know if you prefer or require a specific language.
